Introduction

There is no alignment between the recognition of the IMDs as suitable candidates for immigration as skilled workers and the recognition of the IMD's credentials by the medical regulatory bodies. The immigrant doctors who expect to come and seek greener pastures in Canada find the conditions in Canada not favoring. They are first welcomed with too many conditions, which are strict and at the same mandatory for them to acquire a position in the medical fraternity in Canada.

These doctors must have spent long years in their countries studying, then working and getting experience, a dentist for instance coming from a country like Kenya in East Africa, very qualified and experienced will be treated, when she or he gets to Canada as a skilled immigrant he or she will be treated like a fresh graduate.

The long training and experience the doctor has will not be considered at all. This is a frustration to the skilled immigrant (Singh et al., 2017). The immigrant is supposed to undergo through a series of stages which involve training and getting experience again for him or her to be accepted as a doctor in Canada. Medical professionals from various levels are treated and put on the same level. A General practitioner will be placed in the same category as a gynecologist.

They are made to start all over again from a medical school. They must undergo residency training lasting up to five years so that they can be allowed to have a license. If that is not enough these immigrant doctors who have practiced in their country of origin have to be put under a series of examinations which they must pass for them to be licensed.

The residency training makes it a condition that one must qualify for various tests; NAC-OSCE (The National Assessment Collaboration - objective structured clinical examination), MCCEE and MCCQE1 tests. These tests are not taken by indigents, medical doctors. With this, the immigrant doctors feel very segregated.

There are also several students graduating in medical schools but regardless of their inexperienced or short experienced nature, they compete with the same position with qualified and experienced immigrant doctors (Vandyk et al., 2017). The medical doctors who did not graduate from the Canada medical schools are given few positions.

According to CIC, the number of doctors migrating into Canada is large. Over 2000 skilled doctors are expected to flock every year. Some come as provincial nominees while others are coming to Canada via miscellaneous immigration programs. This translates to many immigrant doctors in Canada. But if we consider the strict rules and regulations they must follow, only a few numbers of them will finally get employment in Canada.

This kind of treatment is tough on the foreign doctors who because of lack of alternative the tests which they pass. But there is more, these doctors must pay heavily to study and register for these exams. The examination fees have been increasing (Creese & Wiebe, 2012). Beside these fees, there are also other expenses of registration and administration fees that are associated with these examinations. Most of them will not have a problem taking these exams if they are assured of getting a job at the end of it.

Another bad side about these examinations is that they are not done all once. One must undertake one, wait for results and given a waiting period before applying for another exam. The waiting period is about one year. This is a long time and considering that the doctor must take more than one exam which translates to more than three years of examinations and tests before getting employed.

According to human capital theory, human beings are made productive and efficient by giving them knowhow through education. This is best achieved when after education one gets to earn from what he or she is learned. Immigrant doctors are not considered as equipped enough with knowledge to perform their duties. (Reitz, 2001; Samers & Snider, 2015). This is a form of discrediting education systems from other countries.

It is also the unfair treatment for a student who has graduated from a prestigious university of the same standard with Canadian university may be of an even higher standard is also expected to undergo the stages of registering and sitting for exams be given a license. This is a factor that demoralizes most doctors and even limits them from coming to Canada to practice.

The government also works in a way through immigration agencies to discourage the immigrants to join Canada health care system as personal support workers or assistants to nurses. This is a joke gone too far as one cannot imagine, being a qualified doctor working as an assistant to a nurse.

Just like any other fresh visitor into a strange country IDMs are usually manipulated and duped in so many instances. The immigration activity has been turned into a business where the Canadians exploit the immigrants who are too vulnerable.

These factors of very strict and tough rules will make most change their careers. Some opt to work first to raise the money required for fees and will find themselves later in life working in other sectors different from their profession or even doing business.

Immigrant Doctors Are Employed in Different Fields to Survive

While the doctors are expected to be taking their expensive Canadian licensing exams, they are also supposed to support themselves and their families by working in other fields for which are usually overqualified and paid the minimum. Most immigrant skilled doctors come to Canada with the idea of getting a well-paying job. They are aware of the medical worker's shortage in Canada and hence they tend to believe there are ready opportunities and vacancies for them to fill.

But as soon as they come to Canada for the green pasture they are faced with a completely different scenario where they have to struggle to get a license. As a foreigner they must undertake various tests, pay various fees and pay for their upkeep. These hardships will compel most of them to take up other jobs awaiting qualification and licensing.

The series of exams subjected to them take a period of three years to complete. Taking and qualifying for the exam is not the end of the struggle, there is a lot of competition between the indigent graduates and the immigrant skilled doctors. Due to this competition being not every immigrant gets a job. most of them are forced to opt to just pick any job that comes their way. Hence, they work in areas that they are overqualified but sadly they are underpaid.

The immigrant professionals are working as cleaners, as taxi drivers, as cooks, some as teachers, as security personnel, others are on manual labor on the farms and delivery persons. These they do to survive and to pay for bills.

Dual labor market hypothesis applies even to the immigrant medical doctors. According to the theory of the dual labor market hypothesis irrespective of the qualification one has one is paid according to the segment of the market they are working in (Samers & Snider, 2015)). This means that an immigrant surgeon who secures a job to do teaching will be paid in the category of a teacher or even less now that the person is a foreigner.

One way in which the immigrants manage to survive is to seek jobs that can assist in surviving through bills in Canada while awaiting qualification and licensing. In Canada, job opportunities are available in various sectors like sectors of agriculture, education, industry, hospitality. The unemployed immigrant doctors can seek employment from these areas and serve as a teacher, as cleaners for their survival.

IMDs might leave to other countries after getting their legal citizen status especially if they fail to complete their exams due to financial or social burdens. There is a possibility of IDM leaving Canada to seek for other opportunities elsewhere using Canada citizenship. This Is one situation where an immigrant can use the citizenship to his advantage.

This loophole can be used by some people who just want to gain citizenship and use it for their own movements and other gains (Campbell-Page, 2013). Hence the Canada government might have seen this and imposed strict directives to ensure that ill motived persons do not misuse the opportunity.

Even After Passing All Exams IMDs Are Offered Only a Few Residencies

Even after passing all exams IMDs are offered only a few residencies and jobs and do not have the right to apply to Canadian graduate's jobs and residencies. Another crisis arises when the immigrants fail to secure a job even after having met all the conditions of exams and registrations and licensing.

Even though Canada is facing a crisis in a shortage of physicians, there are a lot of immigrants flocking into Canada in search of green pastures. Persons qualified and skilled in various professions are coming to Canada to find fortunes (Vandyk et al., 2017). Also, persons are using multiple ways to get to Canada. This can only mean one thing; a lot of immigrants seeking for jobs. However, it is a disappointment to them as they are not allowed to enter and compete with the indigents graduates. After going through tests and examinations one will expect to be offered an opportunity to work and be given same treatment as they have incurred the cost of paying and studying for exams and paying for the license at the end.

Labour Market Segmentation (Samers and Snider, 2015; Sinclair, 2017). Due to the high number of people looking for jobs, most would not be lucky enough to get a job within their profession. Hence seeking an alternative job is the only alternative. But as labor is readily available and opportunities are few, these skilled immigrants are forced to find an alternative. The alternative will even be a manual job that will not consider the education level or qualification one has.
