Introduction

The old person evaluation process consisted of different sections aimed at evaluating the health and well-being of a 75-year-old man in the home setting. The questionnaire, through the questions asked in different sections, was meant to obtain information essential in the determination of health factors as well as the social condition of the older generation in different settings. The questionnaire begins by addressing different basic questions such as the participant identity, the interview date, age of the participant, the setting or the place of interview, and some of the basic information meant to test the memory of the participant. The questionnaire also consists of the demographic section, social support section, the financial section, physical health section, psychological health section, and the physical health section. All the sections in the questionnaire are created to help in the acquisition of specific information that can then be used in the analysis of the qualities of the old generation. Also, there is the home safety checklist that aid in the evaluation of home safety; it incorporates different aspects that aim at establishing the conditions of different places within the selected home.

Preliminary Cognition Section

The preliminary section of the questionnaire was directed towards testing the memory of the interviewee. The questions raised mainly revolved on basic information regarding the respondent as well as some external information. The respondent got the correct answers on almost all the questions. The above scenario indicates that the respondent still has a clear memory on some of the basic information. With the calculation, the total errors were 5, half of the total errors expected; this shows that the 75-year-old respondent still had a clear recollection of ideas and a creative mind. In other words, the above scenario indicated evidence of quantitative skills. The respondent was also well updated with the current information.

Social Support Data

In the social support part of the questionnaire, the responses indicates that the respondent has a well-defined social life, there is the constant communication with the relatives, and the respondent also lives with his spouse. Therefore active social life is evident from the information given. Again, the information is given shows that the respondent requires minimal support, a situation that shows that he is still socially strong and capable of doing some of the basic things in his life. In most cases, old age comes with the inability to undertake some of the basic things in life, mostly communicating and keeping in touch with other members of society.

Financial and Demographic

From the responses on financial issues, the respondents are financially organized and needs minimal support for financial activities. He is capable of paying the bills but only burdened by the health coverage. The above section was meant to test the capacity of the respondents when it comes to the handling of financial issues. In many cases, older payment often has the problems in taking care of their financial resources and instead involved third parties in the processes of management. The capabilities shown by the data given in the financial section coincide well with the quantitative abilities presented in the preliminary cognition section.

Psychological Health Data

The psychological health of the participant is positive; from the information given, there is a clear indication that the respondent enjoys every bit of his life as he ages. The above scenario is supported by the presence of the spouse and constant contact with the family and friends. The respondent is happy all the time and little disturbed by the sleep. Even though the respondent feels happy, there is a need for mental health support; this indicates that there is some deterioration in mental capability.

Physical Health Data

While assessing physical health, there is always the need to analyze the respondent's capacity to engage in physical activities (Lachman & Agrigoroaei, 2010). The above scenario is well incorporated into the questionnaire. From the data given, the client seems stable even though there is an increase in age. The respondent is still physical fit given that fact that he does not require any support while walking. The overall health of the respondent is good, and the presence of the supporting nurse is not necessary. Again, the respondent does not suffer from some of the complex diseases that may interfere with the general health.

Activities of Daily Living Data

Data given in the above section indicates that the client can perform some of the basic tasks independently indicating that he is not troubled with the old age. However, the respondent is incapable of undertaking numerous household chores, which is common even among the young and middle-aged generation. The above scenario indicates that the respondent is still physically fit to undertake some of the basic things in life.

Evidence-Based Health Promotion

From the respondent's data, one of the health promotion strategies is the oral health outreach program that would help in enhancing physical endurance as the age increases (Courtin & Knapp, 2017). Health education strategies may also be standout to be an effective evidence-based practice that may ensure the promotion of social and mental health status. Undertaking basic and continuous exercise programs will enable the respondent to extend or perpetuate his mental stability (Kadaluru, Kempraj, & Muddaiah, 2012). Proper balanced diet and healthy eating is an evidence-based health promotion that would enable the respondent to prolong both their psychological and physical health. Finally, effective treatment of the disorder, particularly the digestive disorder as evidenced in the responses is a practice that will ensure the promotion of health of the respondent.
