Introduction
There has been an increase in the rate of social isolation among the elderly. This has posed a significant challenge for both the elderly and the community. Incidences of social isolation have seen to cause depression and in most cases, death to the elderly. The focus of my research is to find out what are the causes of social isolation, especially to the elderly and try to come up with possible solutions to deal with this issue.

Social isolation is and has been a public health issue that affects people of different age brackets, but most specifically, it has had a hard hit in the elderly within the society. The elderly are more susceptible to the problems of social isolation as compared to the younger segment, and this result in health inequality. Some of the factors that cause health inequality among the elderly include; limited mobility, death of partners and peers, reduced social networks, variations in social roles either due to loss of income or retirement. Also, social isolation is in one way or another linked to health disparities (Goonawardene, Toh, & Tan, 2017). The elderly are more likely to be uninsured due to their lack of income. Thus, they may face a lot of barriers in accessing health care services as compared to young individuals who are earning a salary. Due to lack of access to healthcare services, the elderly experience worse health outcomes leading to higher mortality rates.
The primary community resources and primary prevention resources currently in place to address the health concern.For older adults, social isolation is not just a mental issue; instead, it is deeply linked to physical health. Unfortunately, it has become a growing trend. There are various resources to help reduce social isolation among the elderly. First, to learn if someone you love is affected by loneliness, one can use the AARP self-assessment tool. Second, volunteer opportunities made available by the community to the elderly, help them to meet and interact with others as they help the community. Third, the elderly are connected with caregiver support and enrichment programs, and this helps to reduce isolation. Lastly, since it is based on a model of wellness and health, the community helps the elderly to remain independent. Continuing care at home membership is one way to keep the elderly remains socially active, and at the same time, reduces isolation. Various activities, services, and other useful resources like; companion services, and health risk assessment classes, nutrition review, and a vigorous calendar of social events help deal with the issue of social isolation.
Discuss the Underlying Causes of the Health Concern
In many instances, older adults experience aging as a favorable time, and this is because they remain active and connected to other people. On the other hand, other elders gradually become disconnected from the community, friends, and even family members. The elderly consist of the fastest growing segment of the world's population. As people are living longer, the number of elderly persons increases over time.
A range of factors contributes to social isolation. Therefore, an ecological approach becomes essential in understanding the dynamic and complex relationship between the elderly and the environment. Environment plays a significant role in causing social isolation among the elderly (Landeiro et al., 2017). This is because most social settings are not aging in a friendly way. Elderly people prefer to age in place and to achieve this objective, both the urban and rural community has to adapt. Additionally, milestone events like retirement may represent a momentous transition in life and may lead to isolation. Retirement forces people to create a new plan out of their normal way of life. On the other hand, retirement may come as a shock to other people. Hence, retirement reduces the degree of social contact, and most people tend to close ties with friends leading to social disconnection, thus translating into social isolation.

The campaign shall, therefore, be effective through the setting of S.M.A.R.T social media goals.

These interventions are population-based for the elderly, who could potentially be impacted by social isolation. The two main interventions in our case include education and health promotion. Education is one key way of effectively communicating any crucial information. When the society is educated on some of the causes, effects, and consequences of isolation on the elderly, they would be able to view the matter from a different perspective. Most young people do not understand the challenges faced by the elderly, and thus, they are unable to relate to their grievances (Domenech-Abella et al., 2017). Educating the younger generation on the importance of taking care of the elderly and taking time as well to volunteer and spend more time with the old will help in reducing cases of social isolation.
On the other hand, health promotion is another important intervention. Health care social media has become useful to many. Through this platform, they can connect and share similar experiences. Social isolation has caused a series of major diseases to the elderly, and as they get to interact through the Health care social media, they create friendship bonds. Also, the elderly learn of physical activities and healthy eating habits. By the implementation of these interventions, not only will it help reduce the rate of social isolation, but also it will offer additional health benefits to the targeted community.
