Introduction
Caring for the old is one area that demands an interdisciplinary approach because it cuts across all spheres in the healthcare system. It is the issue from the interview that I chose to write a report about. Caring for health in the health sector is one area that could be improved by all health professionals. The essay will mainly look at the mentioned heath issue and the best practices with which interdisciplinary approach can take. Caring for the old is the health issue that will be discussed in the report and a detailed interdisciplinary approach to improving the situation.
An Interdisciplinary approach is one that involves team members coming from various disciplines operating as one with the main focus of setting goals and then distribute the multiple responsibilities and resources available. A disciplinary approach takes the hand of a clinical team coming from various health disciplines and with the assistance of the patient, they undertake diagnosis, assessment, and intervention of setting strategies for the creation of a care plan.
An interdisciplinary approach was found to be vital in the caring of the old age because the old often demand several diagnoses and various complex needs. It is essential to improve on the need of the old because compared to the young generation, a bigger group of the old need an interdisciplinary means to their care to deal with their complex psychological issues. An interdisciplinary approach is also very crucial for the care of the old because the best outcomes for them can only be met if a collaborative and consultative means for care are satisfied with the patients.
An interdisciplinary approach is beneficial to the old, mainly to avoid the risks that would otherwise rest on the patients. An interdisciplinary for the care of the old is one measure that can significantly impact on the care of the old. It is an approach that has been proved to improve the outcomes of old care, the health care processes, and also in-service satisfaction. Interdisciplinary also reduces the length of stay and in avoiding assessment duplication, which therefore translates to a holistic and comprehensive record.
The practices that interdisciplinary approach can take in mitigating and improving on the care of the old include the following:
Leadership is one of the significant methods that can be employed to ensure that there is improved health care for the old. Positive management and leadership give a clear vision and direction for the health care team through ensuring that the required resources. Ensuring that the various infrastructures that are needed for the training and also the skills and personalities among the team members. The other approach through which leadership can significantly impact the interdisciplinary approach in improving health care for the old is through promoting an environment in which additional contributions are met through consensus (Ragin, D. F. 2015).
The other practice that could best improve old care through an interdisciplinary approach is through teamwork. An interdisciplinary approach depends on professionals from various sections alongside the patients operate together as a team, for a team to work effectively in the best way they have to ensure that the team members set their strategies and objectives with an understanding of the responsibilities within the structure of the team. The teams should engage in joint assessments and diagnosis and setting of goals.
Communication is also an inter practice that can be employed in an interdisciplinary approach for ensuring improved health care for the old. Communication involves communicating openly to encourage the available genuine collaborations and the breakdown of communication between professionals and a common factor in the health centers. These actions to ensure that communication is passed correctly are meant to ensure that there is improved service delivery in the care for the old. It is necessary that assessments are documented and there is a clinical handover of the various documents that are completed in the health centers (Nelson, R., & Staggers, N. 2016).
The person-centered practice is another interdisciplinary approach that ensures well-coordinated and integrated care that would be based on the needs of the patients. The person-centered approach involves the formulation of shared standardized interdisciplinary practices for the contribution of a holistic and comprehensive person-centered care. The method involves patients in all fields and their empowerment. The above methods are needed to ensure that the interdisciplinary actions taken towards improving the health care of the old are met.

Interdisciplinary practices are strategies that every health center should emulate in the process of ensuring that their patients get the best care. Old care is one field that was a significant focus in providing that the patient receives improved care through the application of interdisciplinary practices. Various strategies that are incorporated with the interdisciplinary practices such as ensuring best communication in the policies that are involved. Health care is a field that has been faced with significant challenges, especially in the case of improving the care of the patients. Every patient deserves excellent health care, and therefore, every health facility should be ready to employ the various interdisciplinary practices that would ensure that patients meet the best care they deserve.
