Introduction
In the nursing sector, there are so many things that take place as far as the health of patients, the way they are handled and the kind of medication given to them is concerned. It is, therefore, so much important for all nurses to make sure that they can understand the various conditions and or clinical issues that are present in their patients. This is all to take care of these patients to improve their health. In the case of Mr. M and all that he has been going through, there are quite several issues that a nurse needs to know before they can carry on with the process of providing medical support for Mr. M.
The nurse being informed about the specific issues that are affecting this 70-year-old man will be essential to avoid complicating his health status more. The medication that is administered to Mr. M should be the right medicine that will be of good use to make his condition better as far as health is concerned and this is the reason why the nurses should be clearly aware and knowledgeable about all the issues ranging from hypertension, appendectomy, hypercholesterolemia and tibial fracture which have made his health to really deteriorate.
Discussion of the Medical Manifestations
From the medical history of Mr. M, he has a combination of health problems which all have manifested the signs at once and thus making it difficult to understand what he is suffering from. From the information presented in the case scenario about Mr. M, both primary and secondary medical diagnoses are likely to be for this man. The signs that he has displayed show that the brain has been affected to a level that he is losing memory ("A Clinical Study to Evaluate Ocular Manifestations of Trauma", 2016). This is because he is not able to identify his family members, getting lost and wandering around at night among other issues. This is the primary thing that is likely to be in this man that his brain has problems since the temperature is average, the weight and other things that were tested in the laboratory.
The secondary diagnoses for this man are that it is likely that there are some serious and dangerous diseases and complications in his body. These diseases are the ones that are bringing about the condition of memory loss. Therefore, it is either he has been affected by high blood pressure or hypertension, which has got such signs, limited physical activity, hypercholesterolemia and the status post appendectomy (Kang & Jacob, 2016). These are the main four issues that are likely to be facing this man and have caused all the other complications in his health, and therefore, they should be considered first in the testing and dealt with effectively.
For example, Mr. M is likely to be affected by hypercholesterolemia which is a condition that a lot of cholesterol has accumulated in the blood to the extent that one is not able to have free flow of blood and oxygen circulation in the body and as a result, the loss of memory and other related problems come in. On the other hand, lack of proper physical activity is also a pivotal contribution to the issues that are facing Mr. M, the older adult as far as his health is concerned (P.V, 2017). The brain will always get refreshed and kept in reasonable condition when one keeps fit by regular exercises. The reason as to why Mr. M is having trouble bathing himself and requires help at all times is because of lack of exercise which led to tibial fracture and thus the cause of the pain that denies him from doing many things for himself.
In the assessment of the conditions, there are specific abnormalities that are likely to be found in Mr. M. For example; the primary anomaly is that the veins in his body are blocked and thus the blood is not able to flow normally. This is because of the high level of cholesterol in the blood, something which has been manifested in most of his medical records. The other abnormality that is likely to be found is that this man has had past experiences that have affected the brain, although there is no record. Therefore, the mind is not functioning well, and this makes it difficult for him to do most of the things he should be doing well. The psychological effects of these issues on Mr. M are that he is not able to interact well with other people and views things differently and not in the way he is supposed to do. Emotionally, he suffers from a lot of stress, and the physical effects are that he may get hurt in the process of wandering at night or when he gets lost sometimes he may get lost permanently (Kumar, 2018). On his family, this health condition is making it hard for them to live with him well, and they cannot understand what exactly is troubling him or what he wants at times. A lot of expenses being spent by the family on treatment would have been spent somewhere else to do a more beneficial thing. The only intervention that can be put in place to help him would be to find out precisely what is the cause of this condition and deal with the exact issue rather than being unsure of what is affecting him.
Conclusion
In conclusion, the four possible problems in Mr. M are high levels of hypertension, hypercholesterolemia, tibial fracture and brain damage. Hypertension is because of very high cholesterol level and the inability of blood to flow well. The reason as to why the other problem would be brain damage is because he is not able to reason and or think well. The difficulties in performing the other duties that he used to perform earlier before this condition manifested. The necessary treatment will be administered with the knowledge that he has been affected by the four problems listed above, and thus, a lasting solution will be given to improve his health.
